of the right lower lobe, draining in the posterior aspect of the LA. The 4 PVs were electrically isolated by radiofrequency ablation, while sparing the accessory PV because of its small diameter and absence of electric activity in the interior. Accessory or supernumerary PVs can arise during embryologic development, and result in additional, separate openings of the LA with typically narrower atriovenous junctions. Knowledge of their existence can be useful to facilitate ablation, to avoid their inadvertent injury, and to assess the possibility that they might be a focus of automatism. Thus, the importance of image integration technology between navigators and CT/MRI is obvious.

[Left atrial posterior wall and pulmonary vein refractory periods are associated with atrial fibrillation inducibility in a swine model].

INTRODUCTION AND OBJECTIVES Ectopic activity originating inside the pulmonary veins has been associated with paroxysmal atrial fibrillation in some patients. However, the roles played by the pulmonary veins and the posterior wall of the left atrium in maintaining atrial fibrillation are not well understood. METHODS Our aim was to determine whether there is a correlation between the refractory...

How to perform pulmonary vein isolation.

The advent of technology for the delivery of radiofrequency energy via electrode catheters, in the late 1980s, led to the perfection of techniques for the ablation of accessory pathways and classical flutter [1e4]. This has been so successful that these substrates for arrhythmia are rarely seen in the Western world. Two major arrhythmias, however, still remain to be conquereddatrial fibrillatio...

The Atrial Fibrillation Substrate: Pulmonary Veins, Posterior Wall, or Both

Atrial fibrillation (AF) is the most common arrhythmia seen in clinical practice and is associated with a notable increase in morbidity and mortality. Although there have been considerable advances in the treatment of AF, the results of drug treatment and ablation remain suboptimal.1 This is largely due to the fact that there is still little understanding of the mechanisms responsible for the i...

[Cor triatriatum in adults: intracardiac echocardiography].

Cor triatriatum occurs during embryological development when the common pulmonary vein fails to incorporate in the left atrium (LA) and gives rise to a fibromuscular membrane that divides the LA into 2 chambers: a superior accessory chamber where the pulmonary veins enter, and an inferior one, the “true” LA, with its auricular appendage, which connects with the left ventricle through the mitral...
